Cancelation of membership: Cancellations have to be requested 30 days before the next payment date. Any other visits require a $25 day pass. This can be paid to Venmo: @HellhoundStrength Addition Member Responsibilities: All members are responsible for their own equipment. There is a home for everything; please put back all bars, weights, attachments etc. before leaving the gym. Chalk also needs to be wiped down from any areas you were touching. **If you fail to do so there is a $25 fee automatically charged; An email and proof to be emailed to the offender. Closing out the gym: ALL lights must be turned off no matter what time of the day you are leaving. **If you fail to do so; there will be a $30 fee automatically charged; An email and proof to be sent to the offender. ALL doors must be shut and locked up; back, garage bay and the front door no matter what time of the day you are leaving **If you leave any doors unlocked or opened there will be a $50 fee automatically charged for the first offense, second offense is a $75 fee automatically charge, third offense is automatic membership termination. HELLHOUNDS STRENGTH AND ATHLETICS LLC Release of Liability In consideration of being allowed to participate in any way in the Hellhounds Strength and Athletics programs, related events and activities, the undersigned acknowledges, appreciates and agrees that: 1 The risk of injury from activities involved in this program is significant, including the potential for permanent paralysis and death, and while particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist; and, 2 I knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of the releases or other, and assume full responsibility for my participation; and, 3 I willingly agree to comply with the stated and customary terms and conditions for participation. If, however I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately; and, 4 I, for myself on behalf of my heirs, assigns, personal representatives and next of kin, hereby release and hold harmless Hellhounds Strength and Athletics, LLC their officers, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premised used to conduct the event (“Releasees”), with respect to all and any injury, disability, death, or loss or damage to person or property, whether arising from the negligence of the releases or otherwise, to the fullest extent permitted by law. 5 No Competition Clause - I acknowledge that all programming and related content owned by Hellhounds Strength and Athletics is prohibited from resale without consent. Failure to comply is subject to lawsuit. a. For 36 months after the separation of the membership for any reason, I will not directly or indirectly solicit business from, or attempt to sell, license, or provide the same or similar products or services as are now provided to any customer or client of Hellhounds Strength and Athletics. b. I will not use Hellhounds Strength and Athletics existing client demographic and confidential information to solicit and provide quotes and/or transfer business to any competing entity. I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.
